Filename appears on window header

I was dragging a file from an open folder window and it "stopped" on the gray menu bar at the top of the folder window. I cannot move it b/c every time I click on it the file opens. I've tried right clicking...I've tried attempts to "collect" and drag or delete. The file is also in the intended location.

It doesn't matter how many folder windows I open, this file is there - showing in the gray menu bar. (see attachment - the file is an InDesign document showing up underneath "Macintosh HD" on the top of the folder header)

Right-click (control-click) in the grey area of the toolbar (top of window). Select "Customize Toolbar" and then drag the file's icon from the bar.

a little trick on the side: cmd-drag an icon out of the toolbar and its gone like dragging a dock icon out of it.
