Problem with Application Specific Keyboard Shortcuts

Hi all,
I have a problem under the Snow Leopard. Basically it appears that non of my application specific keyboard shortcuts work. I have some setup for both Coda and Safari and none of them work.

I understand that the name of the keyboard shortcuts have to exactly match those in the application and they do. To make matters stranger the shortcuts do very occasionally work for no good reason.

Has anybody else encountered this problem or even better has anybody found a solution?

Question: do any of those menu items end in an ellipsis (3 dots)?

Example: Insert…

If so, did you type the three dots using three periods? If you did, that's actually not correct. The ellipsis is a special character unto itself, typed by holding down the Option key and hitting the semicolon key.

(I've had the same kind of problem with QuicKeys which also requires exact matches to menu commands)
